Disconnecting Your Business Service

Please submit the form below to request disconnection of your Clearwave Fiber provided Business Services.

This form can be used to request disconnection of your Full Account (all services) or a Partial Disconnect (keep your account active, but remove specific services or service locations).

Once your form is received, it will be processed within 30 days of receipt. Upon successful processing of your request you will receive a confirmation email at the email address provided on the form. If additional information is required to process your request successfully, a member of our team will reach out to the contact information provided on this form.

Important Information:

If your account/service is under contract

  • We require a 30-day notice for discontinuing services. If the disconnect date is under 30-days, billing will be ended 30-days from the date of this initial request/form submission.
  • Early termination fees may apply. Please see your contract terms for details.
  • If porting phone number(s) active on your account to another service provider, the line must remain active until the port is completed, or you will lose the number(s). Port requests on a disconnected service will result in rejection of port request.
